A: If you provide your name and contact information, yes, the Title IX Coordinator, Deputy Title IX Coordinator for Students or designee will contact you to acknowledge receipt of your report. The Title IX Coordinator, Deputy Title IX Coordinator for Students or designee may also provide you information about resources, support, and potential next steps. If you do not provide your name and contact information, the Title IX Coordinator, Deputy Title IX Coordinator for Students or designee will be very limited in their ability to contact you and respond to the information provided in this report.
Q: What if I am not sure I want an investigation?
A: Submitting this form will not automatically initiate a University investigation of the allegation(s), barring extenuating circumstances. Rather, this form servers as a method to notify the University of possible Title IX violation(s) and enables the Title IX Coordinator, Deputy Title IX Coordinator for Students or designee to provide information about resources, support, and potential next steps. The Title IX Coordinator, Deputy Title IX Coordinator for Students, or designee will always notify all parties prior to the start of a University investigation.
